Parish form: Add a new family to our subscription list for The Criterion

Parish secretaries or other parish staff may use this form to enter a new family for a subscription to The Criterion. Keep in mind that if you send this form, your parish will be billed for the subscription.

Parishes needing to submit information for more than one household might find it convenient to add the information via an Excel spreadsheet.  Please download the file below, New Parishioner Data, and save it to your local files. After adding all relevant data, please submit it to uca@archindy.org.  Please allow 5-7 business days for the information to be added to our database.
